Student Financial Services - Online Forms

Students needing to complete the following documents may do so by utilizing the online form process. Students can click on the necessary form and after submitting to the Office of Student Financial Services, a confirmation e-mail will be sent to your Mercyhurst account verifying that you have submitted the document and that it will be processed.

Please note that before any form can be properly submitted, you will need your Mercyhurst College ID number and you will also be asked to submit your Mercyhurst e-mail address.

Please note that you must have an active Mercyhurst email account in order to access the forms below as you will be asked to log in using your email username and password. Also, you will need to type "MERCYHURST\" before your username as in the screenshot below.

Copies of these forms are also available for download under the "Forms&Documents" section on the Student Financial Services main page as well as stopping into our office.
